Medical Summary DVM Intake Exam Estimated age: 10yrs Microchip noted on Intake? neg History : o surrender Subjective: Observed Behavior – allows all handling, slightly nervous, but no aggression Objective P = 120 R = 36 BCS 6.5/9 EENT: Eyes clear, OS oris atrophy, nuclear sclerosis OU, ears clean, no nasal discharge noted Oral Exam: halitosis, moderate to severe dental tartar and moderate gingival recession PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, no palpable MGTs MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, approx 4cm purplish, soft mass on caudal aspect of L HL next to popliteal ln, non painful , limited ROM in HLs when hips manipulated, but non painful CNS: mentation appropriate – no signs of neurologic abnormalities Assessment overweight severe dental and moderate periodontal dz mass: r/o benign vs. malignant OS iris atrophy limited ROM in hips: r/o OA, stiff from stress, other Plan ok for adoptions long term rec cbc/chem/t4/ua as general health screen, rec aspirate/cytology of mass vs. lumpectomy and histopathology, consider 3v met check prior to any surgical procedures rec joint supplements Prognosis: fair to good SURGERY: Permanent waiver due to geriatric status
