I am a spayed female white and tan shih tzu mix. The shelter thinks I am about 10 YEARS old.
I came in the shelter as a OWNER SUR on 03/30/2016 from NY 10303, owner surrender reason stated was NO TIME.

03/31/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 12.6 LBS.
S: BAR older dog with hx of mammary gland surgery (unknown which MG; attending animal hospital is not open, today, for request of records). O: Neuro: BAR. MS: Amb x 4. Integ: firm, enlarged (@2 cm linear enlargement), and minimally ulcerated L caudal thoracic mammary gland. Long nails and some areas of haircoat. Cardiopulmonary:NSF upon auscultation of cardiac valve areas or lung fields. GU: suspect spay scar. GI: soft, non-painful upon palpation. EENT: enlarged and cloudy globe OD; blind in this eye. Enlarged OS. Missing some incisors. Small to moderate amount of dental tartar. A: R/O glaucoma, MGT. Needs grooming. P: Nails are trimmed and grooming of haircoat is planned for today. Recommend rapid plaement for evaluation of possible glaucoma and suspect. Continue to monitor health, while in shelter. Scan negative BARH AMBx4 Friendly,allowed handling Spayed~10yrs Moderate gingivitis/tartar Blind OD, possible KCS Multiple skin tags on back Enlarged mammary on left side, irritated/reddish in color No fleas seen Nosf

03/30/16 17:15 DOG INFORMATION SHEET Snowball was brought from owner’s home where she has been living for the past 3 months. Snowball was found as a stray in December of 2015 before Christmas. Snowball is not microchipped. Owner stated that she is no longer to keep the dog because she does not have the time to continue taking care of it.. Snowball was living with 5 adults and 1 (11yrs old) child. Snowball got along with all members of the household and never showed any signs of aggression. According to the owner, Snowball has never bitten anyone under her care. Owner did not have any other animals so there is no information of how Snowball would interact with dogs or cats. According to the owner, Snowball would like to play and like receiving attention. Snowball was fed both wet food and dry food and sometimes even homemade table food. According to the owner: Snowball know the command “sit”. Snowball walks well on leash. Snowball has been kept in a crate at night. Snowball is housebroken. She does not use the bathroom in the home. According to the information the owner received from the vet: Snowball is blind in right eye and has had some kind of surgery on her mammory glands.

Snowball came into our center as a stray after a member of the public found her as a stray and kept her for three months. Finder reports Snowball stayed with a family of 5 adults and one 11 year old child. Behavior around cats is unknown. Finder reports Snowball is housebroken to go outside and is crate trained at night. She walked nicely on leash for her relief walk, no pulling. Snowball held gaze for three full seconds during Look item, relaxed. She stood still and accepted the touch during Sensitivity item, slight lip lick, ears and tail neutral. Snowball followed at the end of the leash for Tag game, body soft. She gently pulled her paw back on first Squeeze attempt, did not pull it back on second attempt. She showed no interest in Toy, though this may be due to environment as Finder reports she would like to play and liked receiving attention. When off leash with other dogs, Snowball is a polite greeter. She often expored the yard but does not engage in play. Behaviorally, the Behavior Department feels Snowball would do well with an Average adopter and would make a nice family pet. Look: 1. Dog holds gaze with soft eyes, soft body. She allows head to be held loosely in Assessor’s cupped hands. Dog holds gaze for three full seconds. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture, slight lip lick. Tag: 1. Follows at end of leash, body soft Squeeze 1: 1. Dog gently pulls back her paw. Squeeze 2: 1. Dog does not pull back paw Toy 1. No interest. Dog-dog: When off leash with other dogs, Snowball is a polite greeter.
