Medical Behavior: Green,  Age: 14 years,  Sex: Male,  Weight: 17 lbs

DVM Intake Exam; Estimated age: 12; Microchip noted on Intake? n; History : Stray – Reported that finder is the neighbor to the owner and brought the PT b.c of the condition he has been kept in.; Subjective: QARH; Observed Behavior – Gentle, allowed full PE; Evidence of Cruelty seen – Yes; Objective; T = -; P = wnl; R = wnl; BCS = 3.5/9; EENT: Eyes – OR nuclear sclerosis, OS dorsal central ulcer with fluorscene stain uptake, ears mild brown greasy debris.mild pruritis, no nasal or ocular discharge noted; Oral Exam: Severe peridontitis and calculus on all four aracades; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Two descended testicles; MSI: Nails overgrown and embedding in foot pads, Matting on all four paws and on rear end with feces embedded on the fur on the hind end, Ambulatory x 4, skin free of parasites, no masses noted, Overgrown nails embedded into paw pads, Fur coat overgrown; CNS: Mentation appropriate – no signs of neurologic abnormalitiesRectal: Left anal gland full and expressed, rectal tissue inflammed/thickended; Assessment; -Emaciated 3.5/9; -Severe dental disease; -Matting on all four paws and hind-end included embedded fecal matter; -OS Eye ulcer – dorsal central aspect of OS cornea; -Overgrown nails embedding into paw pads; -Mild otitis externa; Prognosis: Good; Plan: Give all intake txs; Apply erythromycin eye ointment in left eye for 4 weeks BID; Apply Osurnia in both ears once only – repeat in one week; Clipped nails and fur on hind end; Placed on TID feeding; Recommend bath and grooming to remove matts; Recommend follow up examination with referral veterinarian regarding eye ulcer, dental disease, otitis externa; Recommend following up with Cruelty team at MACC as these clinical signs show evidence of cruelty.; SURGERY: Permanent waiver due to age
