DVM Intake Exam; Estimated age: 13yrs; Microchip noted on Intake? no; History: O/S due to allergies ;Subjective: Dog is BARH; Observed Behavior – greets veterinarian and allows to be picked up and placed on exam table, nervous, but allowsmost handling with minimal restraint;Objective ;T = NA;P = wnl; R = wnl; BCS 4-5/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Grade 3/4 dental dz; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: tense, no masses palpated; U/G: male intact, both testicles descended; MSI: Ambulatory x 4; Grade 3/4 MPL LHL, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: not performed; Assessment: Geriatric healthy – moderate-severe dental dz, Grade 3/4 MPL LHL; Prognosis: Good; Plan: Recommend geriatric bloodwork, neuter, and dental on placement; SURGERY:Permanent waiver due to age
