DVM Intake Exam; Estimated age: approx. 14; Microchip noted on Intake? neg; History : Stray; Subjective: Q-BAR, well behaved and good for medical handling and examination. Observed Behavior – Objective ; T =; P = wnl; R = eup; BCS 4/9; EENT: OU cataracts, ears clean, no nasal discharge noted. Black crusted discharge OU; Oral Exam: majority of teeth missing, maxillary canines and remaining molars severely diseased, halitosis, protruding tongue; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: tense but non painful, no overt masses palpated; U/G: intact male, 2 testicles descended, smooth, symmetrical; MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt haircoat. Hypotrichosis along bridge of nose, rostral nasal abrasion. Thickened stifles L>R, bilat stifle crepitus, right MPL. Ambulates well, no discomfort noted on manipulation of limbs. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Normal externally. Assessment: geriatric MI 14yo small breed; cataracts; dental disease; MPL; OA; Prognosis: fair; Plan: CTM at MACC until transfer/adoption; recommend full bloodwork and dental post placement; SURGERY: Permanent waiver due to old age
