I am a female white poodle min mix. The shelter thinks I am about 11 YEARS old.
I came in the shelter as a STRAY on 11/05/2016 from NY 10309, owner surrender reason stated was STRAY.

11/09/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 11.0 LBS.
S: observed to be drinking a lot O: BAR OU- large amount of black crusted discharge AU- otic hair severe dental calc and a loose maxillary premolar seen panting and difficult to auscultate but no murmur heard, regular rhythm abdomen- tense approx 1.5 inch soft SQ mass at ventral thorax caudal mammary chains thickened and firm bilaterally no vaginal discharge seen erythema and papules in inguinal area; a few raised crusted areas at caudorsum A: severe dental disease possible polydipsia- R/O DM vs pyometra vs renal disease vs Cushing’s vs other thickened and firm mammary chains- R/O neoplasia vs mastitis vs other soft SQ mass- R/O benign (lipoma) vs malignant dermatitis, possible ruptured cysts at caudodorsum P: BG= 107; CBC/chem machine not currently working cleaned discharge under eyes start clavamox 62.5 mg PO BID x 14 days recommend full bloodwork as soon as possible; recommend dental; recommend biopsy if mammary chains do not improve and/or get worse

11/05/16 16:16 Finder stated that Snow was a bit nervous at first but did allow to be picked up and placed into the car. Behavior during intake: Snow was nervous during intake but did approach me. I was able to scan for a mc, collar and take a picture without any issues. Snow relaxed after a few minutes and allowed me to pet her.

KNOWN HISTORY: Unknown Unaltered, stray SAFER ASSESSMENT: 11/8/2016 Look: 1. Dog’s eyes are averted. Her ears are back, her tail is down, and she has a relaxed body posture. Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, her eyes are averted, and her tail is in neutral position with relaxed body posture. Dog’s mouth is closed for at least a portion of the assessment item. Tag: 2. Dog is not fearful, but is unresponsive to the Assessor, a bit distracted Squeeze 1 / 2: 2. Dog pulls back paw. Toy 1. No interest. Dog-dog 2. Dog approaches helper dog with tail at spine level, body not stiff, ears relaxed, and lip neutral Summary: Snow was calm during her SAFER assessment, no concerning behaviors were seen.
