03/04/2016 PET PROFILE MEMO
3/4/16 13:21 Basic information: Snoopy is a roughly 15 yr. old Shih Tzu mix. The Owner got her from a family member as a gift; he has owned Snoopy for over 14 yrs. The owner surrendered Snoopy because Snoopy is too old and he no longer has time to care for him. Snoopy has trouble seeing (eyes maybe cloudy); he has never been treated and has never been taken to a vet. Socialization: Around strangers Snoopy is friendly playful. Snoopy has not lived with children but he is relaxed and playful around children and adults he does not know. Snoopy has never been around cats or other dogs so the owner has no idea how Snoopy would behave around them. Snoopy has never bitten anyone. Behavior: The owner stats that Snoopy is not bothered when some moves his food or treats while he is eating, when someone takes his toys away or when someone disturbs him while sleeping. Snoopy has not been groomed by the owner. He is okay with taking bathes but has never had his nails trimmed. The owner does not know how Snoopy is around loud noises such as storms or fireworks. For a new family to know: The owner describes Snoopy as friendly, affectionate and playful with a medium activity level. His favorite activity is to walk outside with his owner. In the home Snoopy tends to follow the owner around the house and is kept mainly indoors. Snoopy sleeps in the floor of the bedroom on a dog bed. Snoopy was fed various name brands wet dog food twice times a day (can’t eat dry food due to missing teeth). He is house trained and rarely has accidents in the home, (if not walked regularly he may pee in the home) when outside he goes to the bathroom on or around trees and grass. Snoopy does not know any commands. For exercise Snoopy is used to brisk walks on the leash and twice a day and does not pull on the leash. Behavior during intake: Snoopy was relaxed with lose body. Snoopy allowed us, scanned (-), collared and allowed placing in a dog kennel.
