Medical Behavior: Blue, Age: 2 years, Sex: Female, Weight: 43 lbs
DVM Intake Exam; Estimated age:1-2 y; Microchip noted on Intake?n; History :stray – doh hold for killing a cat; Subjective:bar; Observed Behavior – muzzled for safety, but seems friendly and seeks attention from people, reported to be interdog aggression; Objective ; T =; P = wnl; R = wnl; BCS 5/9; EENT: Eyes clear, ears have minimal debris but shows evidence of scratching, no nasal or ocular discharge noted; Oral Exam: muzzled, but overtly good condition; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: no spay scar,although no prominence to vulva or mg’s; MSI: Ambulatory x 4, skin free of parasites, no masses noted, evidence of scratching ears and contact dermatitis left inguinal area; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: ne; Assessment: 1-2 yr fi medium mixed breed dog; otitis and contact dermatitis -r/o underlying allergy vs other; Prognosis:excellent; Plan: placed otomax in ears and on contact allergy area and will continue as treatment for next 7 days and recheck as she is very friendly with people; SURGERY: Okay for surgery
