Medical Summary Microchip: negative Sex: intact female–please evaluate herniated area, possible spay scar over Age: appx 7-8y, teeth appear older but could be due to diet and poor living conditions Mentation: BARH Eyes: clear Ears: waxy, cleaned with ear cleaning solution Nose: scaling but no d/c Teeth: severe underbite, moderate-heavy tartar with staining, some wear on exposed incisors If abnormal BCS: underweight, 1.75-2 Skin: slightly prolonged turgor, otherwise WNL Hair Coat: some feces staining on back end but otherwise WNL Declawed: N/A Any injuries: no; dog appears to have diarrhea due to malnutrition vs parasites–watery sample, could not pick up off of floor, parvo test (-), possible vaginal d/c but could be moist dermatitis from “wet tail,” umbilical hernia and possible inguinal hernia (soft mass over bladder area, not painful, possible intestinal loops palpated); dog vomited afterdrinking (volume portions needed) Behavior: friendly, quiet, allowed all handling and sought out interaction Medication: N/A, withholding pyrantel (dewormer) until stomach acclimates to feedings, activyl given
