DVM Intake Exam; Estimated age: ~ 8 years; Microchip noted on Intake? negative; History : o/s ; Subjective: BARH. Pollakiuric; Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures; Objective P = wnl ; R = eupneic ; BCS 5/9; EENT: Eyes clear with nuclear sclerosis ou, ears have moderate waxy/yeasty debris, no nasal discharge noted; Oral Exam: adult dentition with mild dental disease, prognathism, no oral lesions noted; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: FI, no MGTs, no vulvar d/c; MSI: Ambulatory x 4, skin free of parasites, no masses noted, moderate yeasty/pyoderma on ventrum CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Pyoderma; Otitis externa; Nuclear sclerosis; Pollakiuric r/o UTI vs TCC vs other; Dental disease; Plan: Continue to monitor while at BACC; Give medicated bath and clean ears tomorrow; Start tresaderm AU BID x14d until 12/5; Start clavamox 13.75mg/kg PO BID x14d until 12/5; Recheck skin and ears in 7 days; CBC/Chem/UA tomorrow; If no resolution of pollakiuria rec AXR and AUS; Prognosis: Good; SURGERY: permanent waiver from surgery due to age
