Medical Summary 03/31/16 14:41 Scan : negative Sex: male, cryptorchid Approx 9y S/O: BAR, nuclear sclerosis OU. Mild serous nasal d/c, no congestino or sneezing. Dental disease grade 2/5. Grade 3 left systolic murmur, strong synchronous murmur . Abdomen tense on palaption. Cryptorchid. A: Geriatric underlying, heart murmur, mild dental disease. P: Recommend NH plea for placement and geriatric workup.CBC/CHem/UA, echocardiogram and chest radiographs. PROGNOSIS: GOOD, short-term. Fair-Poor long-term. Shortly after exam, Skippy boy was outside w/ NH and had a minor fainting-like episode that lasted ~ 5 seconds. Skippy boy quickly recovered and was BAR, w/ normal heart rate a rhythm. Ddx: Vagal response, syncopal episode, focal seizure. Permanent waiver from surgery at ACC.
