Medical Summary Sex: Male intact Scan: Negative Estimated age: 3 years based on eyes, skin on face and teeth Fleas: No flea dirt. S: Growls and lunges, or barks with tail up (not between legs) at most of the men in the clinic, but is extremely quiet and friendly with the female examiner. Leaned into me during exam and allowed all handling and vaccinations, tail up or down. O: BAR-H, BCS 2/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, AU, nose. Moderate tartar. PLNs: Not enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male intact, testicles soft and symmetrical. A: 1. Underweight 2. Moderate dental disease Short-term prognosis: Excellent P: 1. Gave 1.5 ml pyrantel and Activyl 14-22# during exam 2. Monitor weight 1088
