Estimated age:8 Microchip noted on Intake? no History : pu/pd reported, losing weight Subjective: Observed Behavior -wags tail, gives paw Objective T = P =80 R =wnl BCS 3/9 EENT: Eyes clear, ears clean, no nasal or ocular discharge noted Oral Exam: minor staining of teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Assessment: must rule out DM due to reported pu/pd and weight loss. seems to be in good health Prognosis:good . if dm is present, fair Plan:cbc/chem. if wnl, monitor weight
A Little Bit About Me…
A volunteer writes: Simmy is a very pleasant and enjoyable pooch to spend time with.He knows his way around people as he was(and still can be) a family dog, very much loved by his owner and her sons. Simmy is at his door, ready to be leashed and to go.. What a great walker he is . I love the way he checks with me for approval , expecting of course..a treat… He is not a bird chaser or a marker and is very civil to all other dogs met. Simmy makes full use of our backyard. He explores(there are so many scents left by his peers) but mostly follows me as he discovered quickly that I am the source of tasty treats. He sits before I ask, looking like a choir boy, so candid, his eyes full of hope…Simmy can also jump high for duck jerkies and even attempts to search my bag hung out of reach…Balls, he loves them, run fast after them and darts through the air to catch them…Simmy is quite energetic although not hyper. He moves with a purpose just when it is needed. He is a bit on the thin side although he is nicely dressed. He wags his tail a lot, owns “talking” ears and lets you know that he appreciates caresses and hugs. Yes, he is a very affectionate guy and he gives paw readily on command. I love my time with Simmy, a middle aged gent who has a lot to offer and will still make for years to come a wonderful, fun and loving companion and forever best friend. Come and meet him soon. Simmy is waiting for you..
