Eyes- blind with what appears to be corneal scarring +/- edema; left eye clear Ears and nose- clean and clear Teeth- Moderate tartar and gingivitis; No parasites or fleas seen Ambx4 left Luxating patella per Dvm Grade 4/6 left systolic heart murmur per dvm Inflammed protruding penis- rinsed debris with NaCl and applied lube. A: Heart murmur Luxating patella dental disease Corneal scarring/edema Px: Fair P: Recommend placement with full geriatric work up, cardiac work up. Waive permanently from surgery at BACC
