DVM Intake Exam; Estimated age: 11 years; Microchip noted on Intake? no; History : owner surrender; Subjective: Observed Behavior – timid, but able to handle easily ; Objective ; P = wnl; R = wnl; BCS 6/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: moderate dental tartar/periodontal disease; PLN: No enlargements noted; H/L: Grade I/VI heart murmur ausculted, CRT < 2, mild to moderate stertor, Lungs clear, eupnic; ABD: Non painful, no abdominal masses palpated; U/G: intact female; prominent vulva with no discharge; small (~1-2 cm in diameter), firm masses felt in the area of the left caudal abdominal mammary gland and the cranial thoracic right mammary gland; MSI: Ambulatory x 4, skin free of parasites, no masses noted, dry, flaky skin noted along the dorsum, all nails appear overgrown; CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: formed stool (reddish in color, but not overtly hemorrhagic); no masses palpated; Assessment; 1) Mammary masses- most likely mammary adenocarcinoma as multiple mammary glands affected and intact r/o metastasis to other areas (pulmonary) 2) Dermatitis- r/o allergies vs metabolic (hypothyroid, cushings) vs less likely parasitic vs less likely immune-mediated vs less likely neoplasia 3) Heart murmur (1/6)- r/o underlying heart disease (MVD) vs physiologic (DRVOTO) 4) Dental disease Prognosis: guarded (long term); Plan: Scheduled for thoracic radiographs and bloodwork for tomorrow; SURGERY: Permanent waiver due to age, heart murmur
