Medical Summary S: BAR, screaming, difficult to handle O: Scan: negative Sex: female, no palpable spay scar Appearance: BARH Integument: Appropriate hair coat, normal nails EENT: OU clear, AU clean, No nasal d/c Oral cavity: MM pink, CRT <2sec, severe dental tartar PLN: WNL H/L: No murmur or arrhythmias, Lungs clear, but auscultation limited by squirming/screaming ABD: Soft non painful, no palpable masses or organomegaly U/G: female spayed, no active vulvar discharge M/S: Amb x 4, no appreciable lameness, BCS 4/9 Neuro: NSF A: PU/PD reported at home- Bloodwork unable to be run due to machine being broken- r/o DM vs/ renal vs. pyo vs. other Thin body condition dental disease P: Continue to monitor for PU/PD. If noted in hosp recommend run bloods +/- abdominal radiograph, no current specific treatment in the absence of specific signs Prognosis: fair
