Medical Summary DVM Intake Exam Estimated age: 10yrs old Microchip noted on Intake? no History : owner surrender Subjective: Observed Behavior – docile, allows all handling, no aggression Objective P = 108 R = 40 BCS 5/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: severe gingivitis and moderate tartar, halitosis PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: FI, no palpable MGTs, grossly normal MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, but mats on face and extremities CNS: mentation appropriate – no signs of neurologic abnormalities Assessment severe dental and moderate periodontal dz FI matted Plan N/T set up for grooming long term rec cbc/chem/t4 as general health screen, also strongly rec a dental px with possible extractions Prognosis: good SURGERY: Permanent waiver due to age (senior)

Little ShayNieves is with us as her ailing owner could not take care of her anymore. In her kennel, she makes herself very small, flat to the bottom of her den, her two lovely eyes peering despite themselves through a curtain of hair. She does not peep a sound… She is so pretty, compact and dressed in a silky elegant and slightly wavy marron place and cream coat. Few mats are noted. She slowly comes to life in the yard, leash free, walking around and doing her business. She likes to be caressed, takes comfort in being on the lap and accepts few treats.. Ah…She so misses her home and the warmth of a master….Little ShayNieves is waiting for her wheel of fortune to turn in the right direction and stop…right in your arms….She is adorable, gentle, petite, fits in a pocket and will make once again a lovely friend and companion. Come and meet her soon at the Manhattan Care Center.
