Medical Summary 02/24/16 17:52 Scan : Negative Sex: female Approx 9-11 yr S/O: BAR. No nasal discharge. Cataracts OU. Ears clean. Severe dental disease, grade 4/4. Extractions needed. Abdomen tense on palpation. Approx 6 mammary tumors, irregular, firm, varied in size. Grade 5/6 Right systolic apical murmur, grade 2/6 left systolic apical murmur. Right luxating patella. A: Geriatric, cataracts, dental disease, heart murmur (likely mitral valve disease), luxating patella. P: Candidate for placement w/ NH rescue. Requires geriatric blood work, evaluation of mammary tumors via FNA or biopsy, dental and multiple extractions. Recommend CBC/Chem/UA, and echocardiogram prior to anesthesia. Permanently waived from OHE surgery at ACC PROGNOSIS: GUARDED
