I am a female blk smoke chihuahua sh mix. The shelter thinks I am about 15 YEARS old.
I came in the shelter as a OWNER SUR on 03/12/2016 from NY 10458, owner surrender reason stated was PET HEALTH.

03/13/2016 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 4.2 LBS.
VC – Geriatric exam: Eating soft food in cage. Normal BM. QARH. MM pk. Eyes – marked lenticular changes OU, incomplete PLR OD compared to OS but no anisocoria noted when pupils dilated, sclera and conj clr. Ears – cln. NND. Oral – marked tartar and gingivitis with some tooth loss. MSI: thin, BCS= 3/9. Amb x 4. Thin haircoat over legs and chest. CV: HR= 120, nsr, nm. L: clr and eup. Abd: SNP, NMP. UG: Female, no spay scar seen. Inguinal hernia on left side approx size of golf ball but soft and non- painful, partially reduceable. A) Severe dental dz. Decreased vision. Slight anisocoria possibly due to primary changes in the eye due to age but r/o possible neuro pathology. Left side inguinal hernia. P) According to memo, hernia has been present for years. Surgery not recommended since no urgent issue and dog is geriatric. But monitor for pain, problems urinating (bladder could get stuck in hernia), GI issues (problems defecating, vomiting – intestines or colon could get stuck in hernia).

03/12/16 14:01 Shakira A1067436 is an approx. 15 year old Chihuahua. She is being surrendered because her owner is currently in and out of the state and does not have time for her. She recieved Shakira 15 years ago after her sister in law passed away. Shakira’s last vet visit was in 2015 and according to the owner Shakira has a hernia. Socialization Shakira is very friendly and outgoing with strangers. She loves to play with small children and is very affectionate. She plays exuberantly with them even at her age. She loves to cuddle with the owner’s 6 yo grandson. She plays gently with other dogs and cats. She has spent time with an AMPT before and she loved to be chased around. She has not bitten anyone or any other animal. Behavior Shakira loves to go outside. She goes potty on her wee weepad near her bed. She loves baths. She tolerates nail trimming. She is not bothered when you take her food bowl, toys or treats or away from her. She is friendly when strangers approach the owner or family member. She cries alot when she is lonely. For a new family to know Shakira is described as very friendly, affectionate, loving, sweet and a tiny baby. She has her favorite bed that was given to her by a famous baseball player many years ago. She sleeps on her bed every night. She follows the owner around the house and loves to cuddle. She was fed dry dog food and her favorite snack is rotisserie chicken. Behavior during intake Shakira was shy inside of her carrier. She came out slowly and allowed us to leash her. He tail began to wagg as we took off her little jacket. She was very friendly and allowed all handling. She was carried and placed onto the backdrop. She was scanned and collared and photographed.

03/13/2016 BEHAVIOR EVALUATION – EXPERIENCE

Shakira came into our center as an owner surrender citing family health issues. It is reported Shakira enjoys playing with small children, very affectionate. Owner reports she plays gently with other dogs of all sizes (has spent time playing with a pitbull) and cats. It is reported Shakira uses the bathroom on wee wee pads indoors near her bed. Owner states Shakira is not bothered if anyone touches her Food, Toys, Treats. She is described as very friendly, affectionate, loving, and sweet. During intake, Shakira was shy and came out of her carrier slowly, but warmed up and allowed all handling. She held gaze for three full seconds during Look item, body low and crouched. Shakira stood still and accepted the touch during Sensitivity item, lip lick. She followed slowly at the end of the leash for Tag game, stopping at each pass, a bit fearful. Shakira jumped her body away during paw Squeeze, so remaining item was conducted using Flank. She licked her lips but did not move away during both Flank Squeezes. She showed no interest in Toy. Shakira did not approach the helper dog, attempted to exit. The Behavior Team feels Shakira would do well with an Experienced adopter who is familiar with small, nervous dogs. She may initially benefit from a slow approach/slow introduction as well as going home initially to a calm, quiet, structured environment so she can relax and know what to expect.
