Medical Behavior: Green;  Sex: Neutered;  Age: 7 years;  Weight: 11.5 lbs

DVM Intake Exam; Estimated age: 7 years; Microchip noted on Intake? scan negative; History : owner surrender; per owner drinks and urinates a lot; Subjective: Observed Behavior – friendly; Objective ; BCS 5/9; EENT: OU-partial protrusion of third eyelid gland, AU-otic hair, yellow debris, erythema, no nasal or ocular discharge noted; Oral Exam: mild/moderate dental calc; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: MN; MSI: Ambulatory x 4, skin free of parasites, protrusion of last rib at left lateral thorax; approx 1 inch pink pendulous mass from caudal left forelimb; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment; cutaneous mass- R/O benign vs malignant; partial cherry eye OU; otitis externa; possible PU/PD- R/O renal disease vs DM vs CUshings vs other; Prognosis:depends on results of diagnostics; Plan: applied osnuria AU, repeat in 1 week; recommend cytology or biopsy of mass; scheduled CBC/chemistry/UA for 10/20/17
