DVM Intake Exam; Estimated age: approx 10; Microchip noted on Intake? neg; History :stray; Subjective: BAR, very sweet and friendly, no signs of aggression, well behaved for PE and med handling. Observed Behavior -Green (allowed all handling) Objective ; T =; P = wnl; R = eup; BCS 3/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: missing all teeth except back molars; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: MI, 2 testicles, smooth, symmetrical; MSI: Ambulatory x 4, skin free of parasites, no masses noted, fair hair coat. Bilateral luxating patellas, thickened stifles. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: normal externally; Assessment: geriatric; intact; underweight; dental disease; luxating patellas; Prognosis: good; Plan: ctm at MACC until transfer/adoption; monitor weight gain; SURGERY: Permanent waiver due to old age
