Medical Summary Estimated age: 12-17 years old based on overall condition Microchip noted on Intake? Scanned neg on LVT intake. Stray, found in a building . Calm, eating food. Allows all handling as long as I move slowly. QAR, BCS 4/9, MMs pink EENT: Cataracts or nuclear sclerosis OU. AD moderate brown discharge, epithelial hyperplasia; AS severe stenosis and yellow creamy discharge visible. Clean and dry nose. Oral Exam: No teeth PLN: No enlargements noted H/L: Grade 5/6 systolic heart murmur (with thrill), louder on L side of chest than right. NSR. Lungs clear, eupnic ABD: Non painful U/G: Male intact, testicles S/S MSI: Moderate muscle wasting in hind limbs, weakness in hind legs. Good patellar reflexes. Slightly long nails x4 feet. Many small mats with artificial grass inside them. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: 1. Likely cataracts +/- blindness 2. Otitis – severe in left ear 3. Severe heart murmur 4. Missing all teeth 5. Matted 6. Muscle wasting hind legs – R/O effects of being geriatric vs. arthritis or other diease in hind legs leading to decreased weight-bearing Prognosis: Poor-guarded Plan: 1. Groom, treat of otitis (clean ears and instill Osurnia AU) 2. CBC/chemistry 3. Recommend chest rads and heart meds 4. Recommend further evaluation of hind legs and physical therapy SURGERY: Permanent waiver due to age and heart murmur 1088
