Medical Summary 03/28/16 12:48 INITIAL EXAM microchip scan positive approx 6 year old male neutered dog BAR, hydrated ears clean AU no ocular/nasal discharge, reverse sneezing on exam — came in with doxycycline from north shore adult dentition with moderate dental tartar build up, mm’s pink and moist CRT 1-2 sec BCS 4/9 heart and lungs WNL on auscultation abdomen soft and non-painful recently neutered – incision C/D/I and healing normally amb x 4, no lameness or neuro deficits noted full haircoat, skin in good condition, no parasites seen PLAN: gave DA2PP, CIV and bordetella vaccines (no rabies vaccine given, has documentation) gave 1.3 ml pyrantel PO applied activyl topically move to isolation start doxy 1.3 ml PO SID x 10 days
