Medical Summary Hx: Diagnosed with CIRDC on 6/1/17, swabbed for resp pathogens – positive for adenovirus, herpesvirus, coronavirus, pneumovirus AND M. cynos. Pt was adopted that day and went home with doxycycline. Returned for exam on 6/3/17 and was given SQ fluids and mirtazapine. Client reports that pt has been coughing VERY frequently since then, has nasal discharge and hacks up white foam. Poor appetite, difficult to medicate. Lethargic. S: Quiet, ears down, coughing continuously, walks slowly, allows all handling O: QAR, estimated 5-8% dehydration, decreased skin turgor, pale pink MMs, CRT < 2 sec EENT: Yellow crusty nasal discharge. Clean eyes and ears. H/L: NSR, NMA. Continuous coughing. Slightly harsh lung sounds – subjectively harsher on left side of chest. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. No skin lesions noted. UG: Male intact, testicles soft and symmetrical. Neuro: Alert and appropriate, no sign neurological deficiencies A: Pneumonia Short-term prognosis: Good-fair P: Send to BP for treatment – oxygen cage may be needed 1088

Handsome is an understatement to describe Scooby, a 9 month old Boxer mix waiting to meet you at the Manhattan Care Center. He is all puppy, although rather well mannered. He is calm in his kennel, a great walker, does his business on the way, meets nicely other dogs and plays with all his heart in the yard. He loves toys and the ball, does not guard his stuff and enjoys very much tug of war. Scooby does not mind a little rest after playing intensely in the yard. He just last at our feet, appreciating caresses. He can sit on command and for treats of course. Scooby is young but has already quite a number of elements of training, is very sociable, friendly , super playful and likes his own kind too. Well, what are you waiting for ?Scooby is a gem dreaming to be yours…for ever.
