Medical Summary DVM Intake Exam Estimated age: 11+yrs Microchip noted on Intake? no History : Dog brought in by police due to owner being in hospital Subjective: Dog is QAR, approx 7-10% dehydrated (increased skin tent, inc. HR) Observed Behavior – Dog seems alert, but struggling to get out of large crate brought in, using forelimbs but does not appear to be using hind limbs and smells of urine and feces. Once able to get dog out of crate used towel under abdomen to assist in walking. Dog would turn head when trying to place towel, but never attempted to snap. Seems painful. Allows petting of head. Muzzled for exam. Struggles for nail trim and blood draw, but otherwise little restraint required. Lays on floor quietly, not interactive or curious. Evidence of Cruelty seen – no Evidence of Trauma seen – no Objective T = 101.8 F P = 120 R = 30-40 with inc. abdominal effort BCS 3-4/9 EENT: Eyes – mild dark crusts at medial canthus OU, NS and/or immature cataracts OU; ears – large amount light brown waxy debris AU; no nasal discharge noted Oral Exam: Grade 4/4 dental dz; offered small amount of canned food, seemed difficult for dog to locate food at first, but seemed to have an appetite once he found the food, eats gingerly, then tries to nose tray away, finished the food when hand fed PLN: No enlargements noted H/L: NSR, NMA, Lungs auscult harsh bilaterally ABD: Difficult to palpate due to dog unable to stand without assistance, bladder large but easily expressed U/G: no testicles palpated; bladder is large but easily expressed MSI: Non-ambulatory paraparetic, can only stand/walk with assistance (towel under abdomen), nails overgrown (by approx 7-10mm), generalized muscle wasting, coat unkempt – urine and feces on hind end CNS: mentation appropriate – CP deficits both hindlimbs, crosses limbs when lifted up, superficial pain present Rectal: wnl; soft stool on glove Assessment: Geriatric underlying – hindlimb paraparesis DDx: IVDD, Trauma, Neoplasia, Open Underweight with generalized muscle wasting, dehydration, severe dental dz Plan: Dog is on hold here due to owner being in hospital (5 day letter sent). Recommend rescue placement ASAP as dog is unable to walk and urinate on his own and should not stay at BACC. Owner could also be counseled on humane euthanasia. Prognosis: Poor-Guarded SURGERY: Already neutered
