Medical Summary 05/29/17 11:06 DVM Intake Exam Estimated age: 12 yrs old Microchip noted on Intake? yes 276 096 100 137 410 History : found in hotel lobby with abother dog Subjective: Observed Behavior – allows exam with no resistance or retraint, very nice girl Evidence of Cruelty seen – abandoned in hotel overnight with another dog Evidence of Trauma seen – no Objective T = P = 140 R = 28 BCS 76 EENT: pigmentation over sclera and cornea, ears clean, no nasal discharge noted Oral Exam: missing incisors, significant dental tartar and calculi PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, inspiratory stertor ABD: Non painful, no masses palpated U/G: intact female, moderate mamm development no masses palpable, moderate vulvar development no discharge MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, alopecia at ventral tail CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: Assessment pug keratitis OU brachycephalic syndrome vs other upper respiratory vs other dental disease overweight Plan no treatment at this time Prognosis: good SURGERY: Permanent waiver due to geriatric age
