History : owner surrender because owner does not have enough time for him; previously adopted from BACC-was treated for demodex in 2014 Subjective: BAR Observed Behavior – friendly and energetic; allows handling; likes to chew!  Objective BCS 4/9 EENT: AD-yellow crusting discharge, erythema, mild inflammation; pruritic AS-mild yellow crusting discharge, mild erythema Oral Exam: mm pk, moist; CRT <2 sec; underbite; mod tartar; chronic wearing on canines and premolars-suspect from chewing PLN: No enlargements noted H/L: no murmurs or arrhythmias; SSFP; eupnic, referred upper airway noise ABD: Non painful, no masses palpated U/G: intact; 2 descended testicles; severe hyperpigmentation, lichenification with mild ulceration along entire scrotum MSI: ambulatory x 4 with no lameness noted small amnt of sticky debris and pink discharge in hair on top of head thin hair coat along dorsal neck folds-does not appear active mild ulceration of paw pad on RF small pigmented skin tag on ventral chest severe pigmentation, lichenification, mild ulceration of scrotum and tip of penis mild dandruff, diffuse pruritus along dorsum CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment otitis externa (worse AD) scrotal dermatitis-r/o chronic licking vs other pruritus-r/o underlying allergies vs other dental wearing Plan cephalexin 750 mg PO BID x 14 days recheck skin after 7 days, schedule neuter if okay rec’d dental cleaning Prognosis: good to excellent SURGERY: temp waiver d/t dermatitis
