Medical Summary DVM Intake Exam Estimated age: 9 yo MC poodle min Microchip noted on Intake? positive – see record History : O surrender – o moving. No known healthy issues for dog; last vet visit 1 yr ago. Subjective: BAR. Observed Behavior – Timid. Stiffens for exam but allows all handling. Objective T = P = 120 R = eup BCS= 6/9 EENT: Eyes mod lenticular changes OU but full PLR, cornea/ sclera/ conj clear. Ears clean. No nasal discharge noted Oral Exam: Muzzled but able to see caudal teeth – mod- marked tartar. PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male neutered MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Overweight. CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: not done but noted soft brown stool in cage, no blood. Assessment 9 yo MC min poodle – overweight Plan: Dental prophy not urgent. Recom weight loss. Prognosis: Good.
