DVM Intake Exam: Estimated age: 9yrs Microchip noted on Intake? no Microchip Number (If Applicable): History: O/S. Sadie was brought in by her owner originally for a euthanasia request. When asked how old the dog was she stated 9, counselor asked what wahappening with the dog the past few days that brought the owner in. She stated two days ago the dog was in her yard and when the owner came out from the house, the dog was on her side shaking. The owner stated she saw a stick and children running away, and thought the kids hit Sadie on the head. Counselor asked if she took the dog to the vet for medical attention and she stated she had not. The owner stated Sadie had been crying at night since then and she could not deal with it any longer. Counselor offered medical resources which the owner denied. Counselor noted the behavior of the dog as Sadie was bright, alert and active during intake, walking around, jumping and urinating normally. Sadie also took a treat from counselor. At this point counselor did not feel ER was warranted and suggested surrendering instead since the owner did not want to take her to the vet or take her home. Subjective: Dog is Q/BARH. Observed Behavior – quiet, but immediately social and allows all handling with minimal restraint. Objective: T = NA P = 110 R = wnl BCS 3-4/9 EENT: Eyes epiphora OU, ears clean, no nasal discharge noted Oral Exam: Grade 1/4 dental dz PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female spayed, spay scar seen MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: Mentation appropriate – no signs of neurologic abnormalities Rectal: not performed Assessment: Geriatric healthy. Prognosis: Good. Plan: Acceptable candidate for adoption or rescue. SURGERY: Already spayed
