Medical Summary DVM Intake Exam Estimated age: 12-15 yr Microchip noted on Intake? none scanned History : stray by good semaritan Subjective: BAR Hydr wnl voracious appetite Observed Behavior – easy to work with, muzzled for ear exam and cleaning Evidence of Cruelty seen – otitis, emaciated, alopecia, matted fur w/fecal material on extremities elbows, rectum Objective T = 100.8 P = 132 R = wnl BCS 1/9 EENT: Mod lenticular sclerosis, AU no malodor mod to severe crusted black debris extending to prox ext ear canal. AD cystic growth extending from AS full circumference hyperlastic tissue exteding from mid ext ear canal wall, debris trapped below. Oral Exam: mult teeth missing, severe dental dz in remaining teeth PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, normal stool produced on admission. No visible parasites or foreign material observed U/G: nm nsf MSI: Ambulatory x 4, skin free of parasites, no masses noted Nails were mild tomoderately long Emaciated w/ prominent ribs and visible vertebra, no palapble sq fat, temporal muscle decreased mass Dorsal midline alopecia w/ scales on skin extending caudal cervical to sacral-coccygel area. Tufts of fur head, rostral neck bilat trunk w/yellow flakes. Ventral abd focally extensive pigment. Extremities elbows and stifles distally matted fur w/ fecal material, also left lat abd ventral fur, matt r lat thorax and drsal tail. Fecal material in matted fur peri rectally. CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: otitis r/o inf, parasites, allergies, other emaciation and alopecia r/o nutritional, endocrine, other mild to mod long nails w/ fecal matted fur severe dental dz Plan: Flushed ears with ear flush Otomax 4 drops au bid x 7 days Tx rquest for ear clean in 2 days Simparica 5.6-11 po instead of topical fand t because of unknown skin dz dx Rec feed 3x day-place pink sign on cage Rec cruelty evaluation Prognosis: Guarded penidng dx and response to tx
