Medical Summary DVM Intake Exam Estimated age: 1-2 years Microchip noted on Intake? positive History : found as a stray Subjective: QAR Observed Behavior – tense/nervous but allowed handling and examination after a few minutes of petting; allowed muzzle to be placed easily Objective T =not taken P = WNL R = WNL BCS 5/9 EENT: Eyes clear, no nasal discharge noted AS-patch of erythema and mild crusting at external canal and extending slightly onto base of ear pinna; vertical and horizontal canals WNL Oral Exam: mm pk, moist; CRT <2 sec; teeth are very white with no significant tartar build up PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: female; no spay scar or tattoo noted MSI: Ambulatory x 4, skin free of parasites, no masses noted, hair coat slightly dirty-shedding due to anxiety CNS: mentation appropriate – no signs of neurologic abnormalities Assessment Healthy young adult Plan no treatment needed Prognosis: excellent SURGERY: Okay for surgery
