DVM Intake Exam; Estimated age:8-10; Microchip noted on Intake? n; History : stray; Subjective: Observed Behavior -shaking badly on rear legs during exam–started on table for likely fear; Objective T =; P =60; R =wnl; BCS 5/9; EENT: ou sclerosis, ears clean, no nasal or ocular discharge noted; Oral Exam: mod staining, underbite; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: intact female; MSI: Ambulatory x 4, severe matting; CNS: Mentation appropriate – legs shaking but likely due to fear; Assessment: severe matting; Prognosis:excellent; Plan:shave; SURGERY: Permanent waiver due to age
