DVM Intake Exam; Estimated age: ~1-2yrs; Microchip noted on Intake? no; History : Owner surrender due to allergies, reportedly started sneezing with runny eyes and nose since visiting petco last week; Subjective: BARH; Observed Behavior – Allows all handling but tense, stays low to ground; Objective ; P = WNL R = WNL BCS 5/9; EENT: Eyes clear, conjunctivitis OU, ears clean, mild mucoid nasal discharge noted; Oral Exam: Mild dental tartar, pink mm; PLN: No enlargements noted; H/L: Grade 3/6 systolic heart murmur, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: Female; MSI: Ambulatory x 4, skin free of parasites, no masses noted, mild generalized dermatitis, overgrown nails; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: CIRDC, heart murmur, dermatitis, conjunctivitis, dental tartar; Plan: Trimmed nails. Radiographs scheduled. Rec doxycycline 150mg PO q24 x 14 days, TAB OU q12 x 7 days. Rec full cardiology work up with placement (echocardiogram); Prognosis: Fair/open pending diagnostics; SURGERY:  Permanent waiver due to heart murmur
