DVM Intake Exam; Estimated age: 4; Microchip noted on Intake?y Microchip Number (If Applicable):981020013614689; History :stray; Subjective: Observed Behavior -snapping, very hard to approach. calmed down with muzzle; Objective ; T =; P =60; R =wnl; BCS 7/9; EENT: eyes are caked with dc from hair–shaved area. possible kcs os but cannot tell until eyes treated. au yellow d/c; Oral Exam: mandibular prognathism. protruding lower canine are discolored; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: neutered male; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; CNS: Mentation appropriate – no signs of neurologic abnormalities; Assessment: conjucntivitis, otitis, possible kcs; Prognosis: good; Plan:this dog will be very difficult to treat; tab ou bid x 10d; mometamax sid x 7 d ; shaved face and ears
