No  Microchip Number (If Applicable):  History : Subjective: Observed Behavior – Excited, allows all handling  Evidence of Cruelty seen – no  Evidence of Trauma seen – no  Objective : T =  P = WNL  R = WNL  BCS 2.5/5  EENT: Eyes clear, cropped ears, severe brown to yellow discharge and crusting AU, no nasal or ocular discharge noted, abrasions on rostral muzzle and dorsal nose- likely from rubbing face on cage door  Oral Exam: Clean teeth, mm pink  PLN: No enlargements noted  H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic, tracheal cough when p pulls on leash  ABD: Non painful, no masses palpated
U/G: MI, sst  MSI: Ambulatory x 4, skin free of parasites, no masses noted, dry, dusty haircoat, shedding  CNS: Mentation appropriate – no signs of neurologic abnormalities
Rectal: Assessment: Otitis externa  Prognosis: Good  Plan: Clean ears and medicate with otibiotic  SURGERY: Okay for surgery

PLAY GROUP :Ronnie was brought in as a stray so her behavior around other dogs is unknown.

4/15: When off leash at the Care Center, Ronnie enters the yard with a wiggly, soft body but full of energy. He is pushy in his solicitations and when he offers a very robust play bow but does listen when the female helper dog corrects him.
