Medical Summary DVM Intake Exam Estimated age: 8 years Microchip noted on Intake? microchip scan negative History : Subjective: Observed Behavior – active, good energy level Evidence of Cruelty seen – none Evidence of Trauma seen – none Objective T = not taken P = 110 R = eupneic BCS 5/9 EENT: ears clean, no nasal discharge noted, nuclear sclerosis OU Oral Exam: severe periodontal disease – gingival recession and thick calculus buildup, missing multiple incisors (one mandibular incisor is very loose) PLN: No enlargements noted H/L: NSR, 2/6 left sided apical systolic murmur, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact with two scrotal testes, soft and symmetrical MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, grade 1 MPLs bilaterally – no lameness CNS: mentation appropriate – no signs of neurologic abnormalities Rectal: small anal glands, no masses or enlarged lymph nodes palpated Assessment ~10 year old male intact dog severe periodontal disease – will need dental with extractions in the future 2/6 left sided heart murmur – suspect MVD Grade 1 MPLs bilaterally – no lameness, not treatment indicated at this time Plan Prognosis: Good SURGERY: waive from surgery Permanent waiver due to geriatric and heart murmur
