Medical Behavior Evaluation BLUE
Medical Summary 2/13/17 Sex: Male intact Scan: Positive Estimated age: Reported to be 13; exam is consistent with this Fleas: No flea dirt. Hx: Owner surrender S: Calm, stands and allows most handling. Moves face away from nasal or ocular exam. O: BAR-H, BCS 4/9, MMs pink and moist, CRT <2 sec EENT: Sticky mucoid ocular discharge OU. Gland of the third eyelid OS is prolapsed and covers most of the cornea. Nasal planum is proliferative, andis covered by a thick layer of tan crusting. Underbite, moderate tartar and gingival recession, tan debris is stuck in the juncture between the alveolar gingiva and the buccal gingiva at the front of the maxilla. AU lichenified skin and stenotic canals, some discharge visible. PLNs: Not enlarged. H/L: Grade 4/6 systolic heart murmur, PMI left side. Snorts and sniffles, quiet lung sounds. Abd: Soft, no pain on palpation, no masses palpated M/S/I: Amb x4. Mild muscle wasting on abdomen. Multiple areas of alopecia on dorsum – skin is pale with some comedones. Some hyperkeratosis and hyperpigmentation on ventral abdomen surrounding the right last nipple – appears to be pruritic. The fornix at the base of the tail is filled with waxy discharge. Several areas of alopecia and hyperpigmentation on the tail including the dorsal tail gland. Long nails – one nail on a hind foot is curling around and close to the pad. Proliferative keratin on the digit pads. UG: Male intact, testicles soft and symmetrical. Several enlarged nipples – right last nipple, left first nipple. A: 1. Heart murmur 2. Dermatitis – alopecia on dorsum and tail, possible small pyoderma on ventrum associated with right caudal nipple 3. Likely KCS 4. Cherry eye OS 5. Otitis externa 6. Proliferative nasal and paw pad keratin 7. Muscle wasting and some comedones on dorsum R/O Cushing’s disease Short-term prognosis: Fair-poor SURGERY: Permanent waiver due to heart murmur, geriatric, multiple conditions. P: 1. CBC/chemistry 2. Trim nails during exam 3. Make treatment plan based on bloodwork 1088

Just reading what his owner had to say about him, I knew I would like Romeo, an elderly gent we host at the Manhattan Care Center. Romeo is a 13 year old English Bulldog,  compact, small and  equipped with a working corkscrew tail as well as  an inquisitive wrinkled square face. His folds are clean but a big cherry blocks his vision on the left side. Walking Romeo on my right is to his benefit… He is a slow but  willing walker and  does his business as soon as we are out of the building. He seems to enjoy the sight of the multiple little dogs sitting in the waiting room. Romeo is fine being outfitted with a scarf and dressed in a warm coat. Being tethered to a wall for pictures does not bother him. He likes caresses, scratches on his nose and back. He absolutely loves treats and chomps duck jerkies with his tiny little teeth. He does not mind being picked up and placed on my lap.  Yes, Romeo is a good guy…. He comes with stellar recommendations from his former family who might have relinquished him into our hands as  caring for the elderly presents challenges  not everyone is able to master. Romeo is slower, naps a lot, is  less playful(although he still wrestles with his owner), does have “accidents” and has some medical issues…Still, there is a loving and determined heart living in his chest. Romeo might not be a spring chick anymore but he has not exhausted all his beans…A great and fun friend he can still be to the new owner fond and knowledgeable of older chaps. Romeo is ready to meet you at the Manhattan Care Center.
