Microchip: negative Sex: neutered male Age: reported 16y Mentation: QARH Eyes: OS growth on bottom lid, heavy porphyrin staining OU, n.scler Ears: clean Nose: no d/c Teeth: severe ddz, odorous with loose insertions and heavy calculus If abnormal BCS: WNL Skin: WNL Hair Coat: WNL, groomed Declawed: N/A Any injuries: owners reported “a hernia, tooth decay and a heart condition” and on exam dog was straining to defecate, had frank blood in stool with fibrous material (fb?) and a tense and painful abdomen; uncontrollable urination (possibly pressure in bowels); fecal (-) and parvo (-); attempted DV xr but patient was painful and hard to position, repeat sedated with DVM in a.m. at on call DVM request Behavior: allows handling but tense and painful Medication: on call DVM req 125ml/15m bolus of LRS IVC and 27ml/hr maintenance with 0.15ml bupren and NPO for a.m.
