Medical Summary scan neg ~12yrs, male intact qarh. nervous but allows all handling. bcs 5/9 severe dental tartar/moderate gingivitis, nuclear sclerosis ou, no ocular or nasal discharge, ears wnl normal thoracic auscultation; tense on abdominal palpation, no abnormalities appreciated several small cysts (sebacious) on dorsum, small mass at medial canthus of left eye; ambulatory x 4 one tick found on dorsum and removed a: geriatric, dental disease, rule out sebaceous cysts vs other, tick removed p: continue to monitor while at siacc. rec full geriatric work up with placement. permanently waived from surgery due to age. good short term prognosis
