Medical Summary DVM Intake Exam Estimated age: 10 years Microchip noted on Intake? no History : surrender Subjective: BAR, hydrated Observed Behavior – a little timid/shy, but allows exam and handling with minimal restraint Objective P = 120 R = 20 BCS: 4/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: severe grade 4 periodontal disease, extractions needed PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: male intact MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: geriatric, severe periodontal disease Plan: continue to monitor at BACC treat dental disease with clindamycin: 75 mg PO BID strongly recommend periodontal therapy, many extractions suspected Prognosis: fair to good SURGERY:permament waiver for surgery due to age/dental disease, although consider neuter at time of dental treatment.
