DVM Intake Exam; Estimated age: 10; Microchip noted on Intake? yes; History : o surrender; Subjective: QARH; Observed Behavior – Very fearful, did not need muzzle, growling some, needed towel for exam; Objective ; T =; P = 150; R = eup; BCS= 4/9; EENT: Eyes clear, ears clean, no nasal or ocular discharge noted; Oral Exam: Marked calculus and gingivitis, Grade 4 Ddz. PLN: No enlargements noted. H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Non painful, no masses palpated; U/G: MC; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat. Slightly thin. Very soft 1 cm SQ mass R ventral chest- no pain or redness. Mildly long nails. CNS: Mentation appropriate – no signs of neurologic abnormalities; Rectal: Assessment; 10 yr MC Min pin. Geriatric – apparently healthy; Marked ddz – grade 4; SQ mass R chest – suspect benign lipoma; Prognosis: Fair to good with management; Plan: Routine intake. Needs dental cleaning – not urgent but recom in next 3-6 mos.
