I am a male tricolor and white chihuahua sh mix. The shelter thinks I am about 10 YEARS old.
I came in the shelter as a OWNER SUR on 08/29/2017 from NY 10312, owner surrender reason stated was NO TIME.

08/30/2017 Exam Type RE-EXAM – Medical Rating is 4 NC – SEVERE CONDITIONS NOT CONTAGIOUS, Behavior Rating is NONE, Weight 23.0 LBS.
Hx: Owner surrender, previous owner reports seizures and a large mass at the base of the tail. Owner has had Red for about 5 yrs. S: Tense, avoids eye contact, flinches every time I touch him. Allows most handling. Resists oral exam. O: BAR-H, BCS 9/9 EENT: No discharge OU. Mild serous nasal discharge. AU: mild yellow waxy discharge, moderate stenosis and lichenification. PLNs: Not significantly enlarged. H/L: NSR, NMA. Eupnic, quiet lung sounds. Abd: Large, no pain on palpation, no masses palpated M/S/I: Amb x4. Large soft mass on proximal dorsal tail – 10-15 cm dia, pink, alopecic. UG: Male intact, testicles soft and symmetrical. Neuro: Alert and appropriate, no sign neurological deficiencies A: 1. Otitis externa 2. Obese 3. Hx seizures – none seen while at ACC, not currently on treatment, no past treatment or workup reported 4. Mass – R/O lipoma vs. other tumor Short-term prognosis: Fair P: 1. Recommend weight loss of 5-10 lbs 2. Clean ears every 3 days, instill Otibiotic AU BID x10 days 3. Recommend FNA of tumor 4. Consider neurology workup for seizures 1088

08/30/17 09:37 Basic Information: Red is a 10 year male small breed mix. According to owner Red suffers from seizures and has a mass growing at the base of the tail. Owner has had Red for about 5 yrs. Previous owner can longer have the dog due to lack of time. Socialization: According to the previous owner, around strangers, Red is fearful. Around adults and children, Red is also fearful and likes to keep to himself. Red will solicit attention on his own terms. With adults and children Red does not like to play but rather collect his dog toys and hoard them in his favorite spot. Red has never lived with another dog. But according to owner Red loves other dogs at the dog park and will play in a relaxed respectful manor. Red has never been exposed to cats so behavior around them is unknown. Owner has stated that Red has bit in the past but never broke skin, red only bit out of fear of a new person. Behavior: Previous owners stated that Red does have accidents indoors if he does not get walked on time, Owner leaves wee wee pads out for Red to relieve himself when he cannot get walked. Red isn’t bothered when his food is taken away. According to owner, Red is bothered if a treat is taken away, red will guard his treat. Red also does not like to be pushed or pulled off furniture or having his nails trimmed. According to owner Red loves to get his coat brushed. Red also has a favorite spot in the house and will spend most that day relaxing in his bed. Red will bark when an unfamiliar person approaches the home, or yard. Red also does this when he sees his owner. For a New Family to Know: Red is described as fearful and independent with a low activity level. When in the home, Red is the type to relax In his bed which is his favorite spot in the house and also his favorite thing to do besides being walked. Owner states one of his favorite things that Red does is to fetch his own leash when he wants to go for a walk. Red was kept mostly indoors and was fed wet Purina dog food. Red usually goes to the bathroom on the grass or on the cement sidewalk when owner has the time to walk red. If red does not get out in time red will relieve himself indoors, but always on a wee wee pad. When left alone in the home, Red is well-behaved. Red does know the cue “sit” . For exercise, he enjoyed a slow walk with the previous owner. On the leash, Red does not pull hard and he would remain by his owner’s side.

DVM Intake Exam Estimated age: 10 years Microchip noted on Intake? n History : owner surrender with possible euthanasia Subjective: patient was very stressed and P.E. was very limited to due behavior Observed Behavior – attempted to bite most of the time however never lunged at anybody. Patient was growling during entire exam. Evidence of Cruelty seen – n Evidence of Trauma seen – n Objective P = 134 R = 38 BCS 7/9 EENT: Eyes clear, ears clean, no nasal discharge noted Oral Exam: patient has several missing teeth however, oral exam was limited PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: both testicles palpable MSI: Ambulatory x 4, skin free of parasites, no masses noted, thin hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment – R/O causes for Siezures – R/O Hypothyroidism Plan – transfer to Manhattan for 24 hour observation Prognosis: Guarded Temp waiver due to siezure.
