06/28/17 14:26 When client took Rambo home, she gave him water and food. The dry food he wouldn’t eat but he drank a lot of water. He did look a little rough and had missing hair over his body and red bumps over his face. He approched us with a tight body but let us collar, pet him and scan him for microchip. We gave him food out of our hands and he was quick to grab it with his mouth but didn’t break skin.

Dog allows head to be held loosely in Assessor’s cupped hands. Sensitivity: 1. Dog stands still and accepts the touch, his eyes are averted, and his tail is in neutral position with relaxed body posture. Dog’s mouth is closed. Tag: 1. Follows at end of leash, body soft. Squeeze 1: 1. Dog gently pulls back his paw. Squeeze 2: 1. Dog gently pulls back his paw. Toy: 1. Minimal interest, dog sniffs toy. Summary: Rambo displayed no concerning behaviors on his assessment. PLAYGROUP: 6/29: When introduced off leash to dogs in the care center Rambo is polite and tolerant when interacting. INTAKE BEHAVIOR Upon intake, Rambo allowed handling. MEDICAL BEHAVIOR: 6/28/17 During his initial medical exam, Rambo was quiet and allowed handling. ENERGY LEVEL: We have no history on Rambo so we cannot be certain of his behavior in a home environment. In the care center, he displays a low level of activity. Scanned negative History : Stray Subjective: Quiet, stays in one place, slightly tense body, allows all handling Objective BAR, estimated 5-8% dehydrated, BCS 3/9, MMs pale pink and slightly tacky EENT: Moderate black crusty discharge stuck in the fur near the medial canthus OU. Clean ears (fur in canals), no nasal discharge Oral Exam: Moderate tartar, gingival recession PLN: No enlargements noted H/L: NSR, NMA Lungs clear, eupnic ABD: Slightly distended, non painful, no masses palpated U/G: Male intact, testicles S/S MSI: Ambulatory x 4 but lies down on the table with hind legs abducted, not underneath him. No patellar luxation. Thin fur coat on trunk. Some matting on legs. Tail docked. Alopecia on dorsal nose/muzzle with a small abrasion at the tip of the nose (cage nose). CNS: Quiet, alert, appears to be appropriate mentation CBC/chemistry – mild anemia (Hct 35%), leukocytosis and neutrophilia (WBC 29.6, neuts 26.25), thrombocytosis (635 K/ul), elevated alk phos (1043), elevated GGT (36). Normal T. bili and protein. Normal cholesterol. Assessment: 1. Leukocytosis, elevated alk phos, elevated GGT – R/O Cushing’s vs. cholangiohepatitis vs. neoplasia vs. other 2. Underweight, dehydrated, alopecia – R/O endocrine dz (Cushing’s) vs. other cause 3. Ocular discharge – needs better grooming Plan: 1. Recommend abd rads, abd ultrasound, further further workup for Cushing’s 2. Start Clavamox 62.5 mg tabs, 1.5 tabs PO BID x7 days, and metronidazole liq 50 mg/ml 1 ml PO BID x7 days – to cover for infection 3. LRS 250 ml SQ SID x5 days Prognosis: Fair SURGERY: Temporary waiver due to underlying disease 1088
