Medical Summary Estimated age: 8 yrs Microchip noted on Intake? No. Brought in by owner, has a history of weight loss, diarrhea, and thyroid disease, but no medical paperwork submitted with details regarding dog’s specific problems. Friendly, social, wagging tail and calm for exam. Allows all handling. BARH. mm=pink, moist, CRT<2 s. HR=90, RR=20. BCS=2/9. ORAL: No oral lesions, good dental condition. Slight calculus. EENT: No nasal discharge, ears clean. Mild mucopurulent ocular discharge and scleral injection. PLN: No enlargements noted. H/L: Lungs clear, no murmurs/arrhythmias, no coughing. ABD: Non painful, no masses/organomegaly palpated. Had explosive diarrhea in kennel when being taken out. U/G: Intact male, both testicles palpable. MS: Ambulatory x 4, no lameness. INTEG: Full, clean haircoat. No skin lesions. CNS: Mentation appropriate – no signs of neurologic abnormalities. A:Approx. 8 yr old dog with very thin body condition, diarrhea. History of GI disease. DDx: IBD, infectious disease, dietary intolerance or allergy, EPI, hepatic disease, other metabolic disease, neoplasia. Mild conjunctivitis OU. P: Rx TAB-H OU BID x 7 days. Rx Metronidazole 250 mg. 1.5 PO BID x 7 days. Panacur granules. 55 lb dose Po q 24 hours x 3 days. Scheduled for lab tests. Permanent waiver from neuter surgery due to age and medical condition. Fair prognosis with appropriate management. May need further workup such as abdominal ultrasound, intestinal biopsy.
