13 yr. old spayed black/white small dog, OS, 21 pounds:
DVM Intake Exam; Estimated age: ~13-14 year; Microchip noted on Intake? positive 076 028 822; History : o/s; Subjective: BARH; Observed Behavior – very sweet. Easily handleable. Did well for all medical handling and procedures; Objective ; P = wnl ; R = eupneic ; BCS 5/9; EENT: Eyes have nuclear sclerosis and corneal pigmentation ou, ears have mild yellow waxy debris and hair in canals, no nasal discharge noted; Oral Exam: adult dentition with mild dental disease, no oral lesions noted; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupneic; ABD: Non painful, no masses palpated; U/G: FS (spay scar noted on ventrum), no MGTs, no vulvar d/c; MSI: Ambulatory x 4, skin free of parasites, no masses noted, unkempt and matted, scaly, and oily haircoat with yeasty dermatitis on all 4 legs with lichenification and alopecia, overgrown nails; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Geriatric; Dermatitis r/o allergies; Corneal pigmentation and nuclear sclerosis OU; Otitis externa; Plan: Continue to monitor while at BACC; Trim matts, trim nails, clean ears, medicated bath; Start simplicef 10mg/kg PO SID x7d until 3/5 +/- extend (will need 5 days beyond cessation of signs) Start benadryl 4.4mg/kg PO BID x7d until 3/5 +/- extend; Rec weekly to biweekly medicated baths; Prognosis: Good but will likely need chronic care for skin condition; SURGERY: spayed

he came to his home from a pet shop and is surrendered due to the family not being able to afford care.
