06/14/17 S/O: Q/BARH. Eating and drinking well. Allows all handling. BCS 7/9 EENT: Severe dental disease, early nuclear sclerosis with mild diffuse corneal scarring, no ocular or nasal discharge, mild otitis externa HL: Grade 4/6 heart murmur loudest at cranial left side, lungs clear ABD: Difficult to palpate due to overweight, soft, non tender INTEG: Generalized dermatitis with several areas of pyoderma especially along dorsum MS: Non-ambulatory, tries to stand up but unable to lift weight or stand with assistance, appears to be able to get around cage, motor and deep pain in limbs, hind limbs slightly splayed, appears uncomfortable at thoraco-lumbar region UG: Neutered A: Owner reported spinal issue, hypothyroidism, heart murmur and dental disease — radiographs and blood work scheduled for today — heart murmur and dental disease confirmed P: Rec rimadyl 12.5mg PO q24 x 5 days. Diagnostics pending. Rec cardiology consult prior to sedation for advanced imaging to work up spinal disease with placement Blood work shows a mild leukocytosis characterized by a very mild monocytosis and neutrophilia, very mild globulinemia, low normal T4 levels (1.2) Difficult to position for good quality radiographs without sedation — unable to appreciate any disk disease

06/13/17 19:10 Basic info: Pup-Pups is an 8 year old small black and brown dog who was surrendered to the ACC due to the fact he has a lot of medical problems. The owner stated that she got him as a gift and she has had him for over 3 years. The owner stated that Pup-Pups have a heart murmur, alopecia, and also a spinal injury. Socialization: Owner stated that around strangers Pup-Pups is friendly and outgoing. The owner state that he has been around children and he is relaxed and affectionate. When playing with adults and children he is gentle but due to his medical condition Pup-Pups does not play. Pup-Pups have not spent time around other dogs or cats so it is unknown hoe he will react. Behavior: Owner stated that Pup-Pups will cry during fireworks and loud noises. If someone were to take his food or treats he may growl. He isn’t bothered if someone takes his toys away. He also isn’t bothered if he is restrained, pushed off the couch, or disturbed while he is sleeping. Pup-Pups don’t mind being bathed or brushed. He also doesn’t mind having his nails trimmed. When someone unfamiliar approaches his house or family member he will bark. For a new family to know: Owner described Pup-Pups as friendly affectionate and mellow with a low activity level. When at home he is in his favorite spot. He has been kept indoors only. He has been eating blue buffalo dry food. Due to Pup-Pups medical status he has accidents often but goes on a wee-wee pad. Pup-Pups has never been crated.

The behavior department believes Pup-Pups would benefit best from placement with a New Hope Rescue who can meet his needs medically and then assess behavior once he is in a stable environment. Force-free, reward based training is advised when introducing/exposing Pup-Pups to new and unfamiliar situations. Pup-Pups appears to be calm, he has allowed all handling in shelter without any issue. MEDICAL BEHAVIOR: 06/09/17 During her initial medical examination, Pup-Pups allowed all handling without issue. RECOMMENDATIONS: New Hope Only _X_Place with a New Hope partner: The behavior department believes Pup-Pups would benefit best from placement with a New Hope Rescue who can meet his needs medically and then assess behavior once he is in a stable environment. Force-free, reward based training is advised when introducing/exposing Pup-Pups to new and unfamiliar situations. Potential challenges: _X_Resource guarding: Pup-Pups previous owner notes that he may growl over food/treats. We recommend that Pup-Pups be left alone while eating, and that food guarding behavior modification steps (available at ASPCApro.org) be utilized if this behavior is problematic in his future home. Nothing should ever be taken directly out of Pup-Pups’s mouth, and any time something is removed he should be rewarded with a high value treat or toy.
