Medical Summary DVM Intake Exam 5/12/17 Estimated age: ~7yrs, male Microchip noted on Intake? No History : Owner surrender due to allergies Subjective: Observed Behavior – BARH. Nervous, struggled with restraint Evidence of Cruelty seen – No Evidence of Trauma seen – No Objective P = WNL R = WNL BCS 3/9 EENT: Eyes clear, ears clean, dermatitis around pinnae, no nasal discharge noted Oral Exam: Moderate dental tartar/gingivitis PLN: No enlargements noted H/L: Grade 4/6 heart murmur, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: Male MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat; intermittent non weight bearing lameness LH, remains in extension, grade 3/4 MPL, uncomfortable on palpation, overgrown nails CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: heart murmur, dental disease, MPL Plan: Rec rimadyl 12.5mg PO q24 x 3 days. Rec cardiology work up prior to anesthesia for dental prophylaxis and orthopedic consultation for MPL. Trimmed nails Prognosis: SURGERY: Permanent waiver due to heart murmur
