Medical Summary DVM Intake Exam Estimated age:14 Microchip noted on Intake? y History : multiple bite history. dog cam in muzzled in basket muzzle covered in human blood from recent incident Subjective: dog appears to have chronic resp issues–panting but stertor and honking cough Observed Behavior – unruly during exam but no attempte to bite Objective p 100 r panting, constant bcs 7/9 EENT: ou sclerosis, au hyperkeratosis, otitis Oral Exam: muzzled but halitosis and ddz apparent PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: spayed MSI: Ambulatory x 4, skin free of parasites,large mass on LF leg about 2″ diameter in carpal area, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: collapsing trachea, suspect chronic respo disease. otitis. dental dz, mass on lf leg Plan: no treatment at this time trimmed nails (very long)and applied otibiotic while muzzled recc FNA mass, +/- rads if resp distress seen Prognosis: guarded long term
