Medical Summary DVM Intake Exam Estimated age:14 Microchip noted on Intake? y History : multiple bite history. dog cam in muzzled in basket muzzle covered in human blood from recent incident Subjective: dog appears to have chronic resp issues–panting but stertor and honking cough Observed Behavior – unruly during exam but no attempte to bite  Objective p 100 r panting, constant bcs 7/9 EENT: ou sclerosis, au hyperkeratosis, otitis Oral Exam: muzzled but halitosis and ddz apparent PLN: No enlargements noted H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated U/G: spayed MSI: Ambulatory x 4, skin free of parasites,large mass on LF leg about 2″ diameter in carpal area, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment: collapsing trachea, suspect chronic respo disease. otitis. dental dz, mass on lf leg Plan: no treatment at this time trimmed nails (very long)and applied otibiotic while muzzled recc FNA mass, +/- rads if resp distress seen Prognosis: guarded long term

PROFILE :

Princess is aggressive towars strangers and will snap if they try to touch her. She has never been around children so it’s unknown how she would react but plays exuberantly with adults. She has spent time at the dog park and is friendly and playful with dogs at first but then begins to growl so her owner picks her up and takes her out. She lives with a cat who she is tolerant of. She has bitten three people, including her owner, and also her owner’s cat. These instances happened over a 13 year period.
Princess is nervous during loud noises such as thunderstorms and will tremble and run around the house. She isn’t bothered when her owner takes away her food bowl or treats and thinks she’s trying to play when it’s a toy being taken away. She isn’t bothered when pulled off of furniture, restrained from something she wants, and awoken from her slumber. She loves baths and brushing and has never had her nails trimmed so it’s unknown how she would react. She barks when someone unfamiliar knocks on her door and is placed in the back room because she tried snapping at a person one time during the same situation. She isn’t bothered when someone unfamiliar approaches her humans.
Princess is friendly, playful, and excitable. She has a medium activity level and when she’s ready to play enjoys chewing on tennis balls. When her owner is home, she follows her around and is kept mostly indoors. She sleeps on her own doggy bed in the hallway and is very house-trained. She always goes on weewee pads or outside wherever she can pick up a scent. She’s well-behaved when left home alone and has never been crate-trained. She doesn’t know any commands and enjoys brisk walks on-leash. She does not pull. Princess eats house-made food (ie chicken, spaghetti, and hamburgers) twice a day.
On intake, counselor had owner collar and hold for pictures due to the nature of intake.
