Medical Behavior: Yellow; Age: 8 years; Sex: Female; Weight: 24 lbs

DVM Intake Exam; Estimated age: reported ~8yrs; Microchip noted on Intake? no; History : owner surrender; Subjective:  Observed Behavior – BARH. Nervous, tail tucked, eyes darting, difficult to pick up, soft muzzle placed for exam, soft growl through parts of exam; Objective ; P = WNL R = WNL BCS 5/9; EENT: Eyes clear, ears clean, no nasal discharge noted; Oral Exam: Moderate dental tartar/buildup, pink mm; PLN: No enlargements noted; H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic; ABD: Tense on palpation; U/G: female, prominent mammary glands; MSI: Ambulatory x 4, skin free of parasites, no masses noted, healthy hair coat, thin coat at caudal dorsum; CNS: mentation appropriate – no signs of neurologic abnormalities; Assessment: Mild dental disease; Plan: Continue to monitor while at BACC. Consider dental prophylaxis with placement; Prognosis: Excellent; SURGERY:  OK for surgery

PROFILE:

Basic Information:

Princess is a small breed dog who was surrendered due to her previous owner moving to a place that doesn’t allow animals and unable to find her a home.

How is this dog around strangers?  Around strangers Princess is friendly and plays exuberantly.
How is this dog around children?  Princess has spent time with children nine years and older around them she is relaxed and playful.
How is this dog around other dogs?  Princess has spent time with small dogs and is relaxed and playful around them.
How is this dog around cats?  Princess will try and chase cats she sees outside.
Resource guarding:  Princess is friendly when someone touches her food/bowl while she is eating or takes a treat/toy away.
Bite history:  Princess has never bitten another person or animal
Housetrained:  Yes
Energy level/descriptors:  medium

Medical Notes

Princess is friendly, when someone pushes/pulls her off of the furniture, holds/restrains her,disturbs her while she is resting/sleeping, gives her a bath, brushes her coat, and gives her a bath. Princess is fearful when someone trims her nails. If an unfamiliar person approaches her family/home Princess will be friendly towards them.
For a New Family to Know

Princess is described as friendly, affectionate, playful, confident, and excitable. When at home Princess will follow around her owners, play with all toys and was kept indoors only. Princess was fed both wet and dry food. Princess rarely has and accidents and will go to the bathroom on the grass or a wee wee pad. Princess is well behaved when left home alone and has never been crate trained. Princess knows the command sit and stay. Princess likes brisk walks and pulls lightly on a leash.

Behavior Notes:

Princess was nervous during intake and tended to avoid staff. Princess allowed staff to collar, scan for a microchip and take pictures with the assistance of her previous owners.

According to Princess previous owner, Princes time with small dogs and is relaxed and playful around them.
11/28: When off leash at the Care Center, Princess mostly keeps to herself in a group of small male and female dogs. She offers appropriate corrections when a sexually motivated male persistently follows her.

BEHAVIOR:

Date of assessment:  29-Nov-2017

Look:  2. Dog’s eyes are averted, body posture is stiff and fearful, tail is low and not moving. Dog allows head to be held loosely in Assessor’s cupped hands.

Sensitivity:  1. Dog stands still and accepts the touch, eyes are averted, and tail is in neutral position with a relaxed body posture. Dog’s mouth is likely closed for at least a portion of the assessment item.

Tag:  1. Dog assumes play position and joins the game. Or dog indicates play with huffing, soft ‘popping’ of the body, etc. Dog might jump on Assessor once play begins.

Paw squeeze 1:  1. Dog gently pulls back his/her paw.
Paw squeeze 2:  1. Dog gently pulls back his/her paw.
Toy:  1. No interest.

Summary:  Princess came into the room calm and relaxed, she was friendly, playful and affectionate towards the handlers.
