Medical Summary 04-25-17 O surrender Male Entire 9yo No MIC on Intake BAR Observed Behavior – Easy to handle, friendly, 3/9 BCS EENT: Eyes open clear, ears – heavy debris, no nasal discharge noted Oral Exam: 2-3/4 generalized dental disease, tartar accumulation, gum recession, dental decay H/L: NSR, NMA, CRT < 2, Lungs clear, eupnic ABD: Non painful, no masses palpated, sl bloated/wormy U/G: Male Entire MSI: Ambulatory x 4, bilat luxating patella L > R 2/4, good ROM hips INTG no masses noted, healthy hair coat CNS: mentation appropriate – no signs of neurologic abnormalities Assessment – – Otitis externa – dental dz – luxating patellae Plan – Intake Rec ear cleanings and topical tx; Rec dentistry/ improved oral hygiene Rec joint supplements Prognosis: Excellent with appropriate care – SURGERY: Temp wave sx due to age

PROFILE :
Prince is a 9 year old Male, Chihuahua SH mix; he came to the previous owner’s home from a breeder and is being surrender due to the owner not having time. Prince has a history of having seizures when he gets overly excited but has no other known health issues or injures and has no recent vet history.
Socialization:
When Prince is approached by a stranger he is shy for the first few minutes and will try to run away. He has been around children, of various ages and usually growls or snaps if the try to pet him. Around other dogs Prince is relaxed and gentle and attempt seek other dog to engage in play.
Behavior:
If an individual tries to touch Prince’s food bowl he will growl and bark. During bathing and brushing Prince is slightly tense but will allow individual to wash and brush him. When an unfamiliar person or family member comes to the house Prince will bark at first but after few minutes and later becomes uninterested.
For a new family:
Prince was described as playful with other dogs, shy and mellow with a medium activity level. Prince is feed and house food 2-3 times a day, and he is house trained and uses the grass and cement 2 times a day. . Prince has never been crate trained, and if left alone in the home he will go through the garbage. When taking off Leash he will wander some but will come if called.
Admission behavior:
When the Counselor approached Prince, he/ slight tense body ears pulled back with his tail between its legs. He allowed the counselor to collar him
